Veteran Seraphim Sister Superior
This is a standard Seraphim Sister Superior model from Warhammer 40,000 (Games Workshop).  The original model's jump pack was replaced with a set of Spyre Hunter wings from Necromunda (Games Workshop) and her sword was replaced with a Space Wolves (Games Workshop) sword.
